Between CCFT & the Cabrillo Community College District
July 1, 2007 through June 30, 2010

CCFT and the District agree to the following in regards to the Managed Health Network (MHN) mental health benefit of the Health Net HMO High Option Plan:
Effective October 1, 2007 through September 30, 2008, the MHN mental health benefit for faculty members on the Health Net HMO High Option Plan will return to a $10 co-pay with unlimited visits.
Faculty members who have paid the higher co-pay of $30 for the MHN benefit between July 1, 2007 and September 30, 2007 will be reimbursed by the District for the difference of $20 per visit. In order to request a reimbursement, faculty members must provide a copy of the Explanation of Benefit Payment form (EOB) from MHN that outlines the services rendered and reflects the $30 co-pay for each visit, to Susan Torres in the Human Resources Department.
